Apply today for our Palliative Care Nurse Coordinator opportunity.Job Summary and Qualifications As a Palliative Care Nurse Coordinator, you will be responsible for the development and implementation of a palliative care program. You will provide guidance, support, and education to healthcare team members in the overall delivery of services related to palliative care. You will establish a palliative care plan for all palliative care patients based upon assessments, patient care standards, and patient and family preferencesYou will consult and collaborate with healthcare team members to develop a plan of care and palliative care recommendationsYou will ensure consistent, complete, and thorough nursing documentationYou will review physician orders related to palliative care, and write or enter orders into the system accuratelyYou will provide timely communication to nurses and/or physicians regarding changes to the plan of care and/or changes in the patient's conditionIf necessary, you will provide basic life support in collaboration with the healthcare teamYou will maintain and review patient records, charts, and documents for accuracy, quality, and best practices related to palliative careYou will provide palliative care data, including the number of consults, referrals, conversions to hospice, and changes in level of care Experience 1 year of clinical nursing experience as a Registered Nurse (RN)Previous experience in Hospice and Palliative Care nursingCritical care experience in a hospital setting - preferred Education Associate's degree in Nursing (ADN)Bachelor of Science in Nursing (BSN) - preferredAdvanced Practice Nurse (APN) - preferred Licenses and CertificationsBasic Cardiac Life Support must be obtained within 30 days of employment start dateRegistered NurseAssociate Degree, or Bachelors Degree, or Registered Nurse Diploma This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
