This multi-faceted position involves coordinating and managing St. David's Medical Center Stroke Program. Serves as the subject matter expert to educate patients, families, employees, medical staff and the community. The Stroke Program Coordinator is responsible for formulating short and long term strategies to ensure the organizations vision, missions and values are carried out through the delivery of high quality cost effective patient care. This position acts as a clinical resource and coordinates clinical management of stroke across the entire continuum of care. The Stroke Program Coordinator is responsible for the initial contact with patients and families during the inpatient episode. Assist in the baseline assessment and documentation of patient physical, emotional, mental, educational psychosocial and spiritual needs. Assist assuring the stroke core measures are initiated and appropriate documentation follows Assures appropriate information to facilitate patient entry into the Stroke Database. Serves as a liaison between physician, nursing staff, ancillary staff, patient and families to enhance continuity of care. Serves as a resource person for RNs, managers and ancillary departments. Utilizes database information to position St. David's Medical Center to offer the best quality and most efficient stroke care to patients. Utilizes database information to analyze efficiency of stroke interventions in our patients. Assist with the maintenance of individualized system-based records documenting patient evolving plan of care. Seeks guidance and direction from the physician (s) based on patient assessment. Assist with clinical research related to stroke; works with physician and clinical research team to identify patients that meet criteria for ongoing and new research projects. Requirements
Experience: Required: Graduate of accredited school of nursing. Three (3) years recent experience in relevant clinical specialty (ED, Neurosurgery preferred). Education: Bachelor's of Science in Nursing required. License and CertificationsCurrently licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.Basic Life Support (BLS) required as per St. David's Healthcare policy Advanced Cardiovascular Life Support (ACLS) certification-required St. David's Medical Center is a full-service hospital located in central Austin, Texas. This 300
bed facility features a 24-hour emergency department, the region's largest Neonatal Intensive Care Unit (NICU), and the internationally-renowned Texas Cardiac Arrhythmia Institute and St. David's Neuroscience & Spine Institute. The full scope of services provided at St. David's Medical Center includes cancer care, heart and vascular care, imaging services, joint replacement and orthopedics, women's services, complex surgical services, and a Bariatric Surgery Center. The facility is home to a 60
